What to do When Death Occurs

We will need to gather information about your loved one, and during our initial conversation we may ask the following;

• The full name of your loved one who has passed away
• His or her current location (hospital, nursing facility or residence)
• Your name, address, and telephone number
• Name, address, and phone number of the next of kin

After we have spoken, we will schedule a convenient time to meet at your home or our offices, wherever you choose,  so we can complete the details of the funeral arrangements. At that time, we kindly ask that you provide the following for your loved one:

• Social Security number
• Date of birth, city and state of birth
• Names of his/her parents, including mother's maiden name
• Educational Information
• Marital Status
• Veteran's Discharge Papers or Claim Number (of applicable)
• A recent photograph of the individual
• Clothing for visitation and interment
• Pre-arrangement paperwork (if applicable)
• Cemetery lot information (if applicable)
